Agreement to Terms

Welcome to SageWife. By accessing our website and using our services, you agree to be bound by these Terms of Service and all applicable laws and regulations. If you do not agree with any of these terms, you are prohibited from using our services.

Services Provided

SageWife provides comprehensive midwifery care including prenatal care, labor and birth attendance, postpartum care, lactation support, and related services. Our services are available to clients in Louisiana, Maryland, Michigan, Rhode Island, Tennessee, Texas, and Virginia, subject to applicable state regulations.

Client Responsibilities

As a client, you agree to:

  • Provide accurate and complete medical history and health information
  • Attend scheduled prenatal appointments and follow recommended care plans
  • Notify us promptly of any changes in your health status
  • Follow instructions and recommendations provided by your midwife
  • Maintain open and honest communication about your care
  • Pay for services according to agreed-upon payment terms
  • Understand that midwifery care is appropriate for low-risk pregnancies
  • Accept transfer of care to a physician if complications arise

Scope of Practice

Our midwives practice within the scope defined by state regulations and professional standards. We provide care for low-risk pregnancies and normal physiological birth. Certain conditions require consultation with or transfer to a physician. We maintain collaborative relationships with physicians and hospitals for consultation and emergency care.

Informed Consent

You have the right to make informed decisions about your care. We will provide you with information about recommended treatments, procedures, risks, benefits, and alternatives. You may accept or decline any recommended care. Specific informed consent will be obtained for procedures such as home birth, water birth, and medical interventions.

Payment Terms

Payment terms include:

  • Full payment is due by 36 weeks of pregnancy unless other arrangements are made
  • Payment plans are available and must be arranged in advance
  • Insurance billing is available where applicable; you are responsible for verifying coverage
  • Refunds for unused services are provided according to our refund policy
  • Additional services (ultrasounds, classes, etc.) are billed separately
  • Late payment fees may apply to overdue balances

Cancellation and Refund Policy

If you choose to discontinue care before birth, refunds will be calculated based on services already provided. Appointments cancelled with less than 24 hours notice may be subject to a cancellation fee. If we are unable to attend your birth due to circumstances beyond our control, a full refund of the birth fee will be provided.

Limitation of Liability

While we strive to provide the highest quality care, midwifery services, like all healthcare, carry inherent risks. We maintain professional liability insurance as required by law. Our liability is limited to the extent permitted by applicable law. We are not liable for outcomes resulting from your failure to follow care recommendations or disclose relevant health information.

Emergency Situations

In the event of a medical emergency, you agree to follow emergency protocols, which may include calling 911 and/or transfer to a hospital. We will accompany you to the hospital when possible and provide continuity of care to the extent permitted by hospital policy.

Confidentiality

We maintain strict confidentiality of your health information in accordance with HIPAA (US) and PHIPA (Canada). Please refer to our Privacy Policy for detailed information about how we protect and use your information.

Website Use

When using our website, you agree to:

  • Use the website only for lawful purposes
  • Not attempt to gain unauthorized access to any part of the website
  • Not transmit any harmful code or malware
  • Not use automated systems to access the website without permission
  • Respect intellectual property rights of all content

These Terms of Service are governed by the laws of the state in which services are provided. For Canadian clients, provincial laws apply.

We reserve the right to modify these Terms of Service at any time. Changes will be posted on our website with an updated "Last Updated" date. Continued use of our services after changes constitutes acceptance of the modified terms.

Severability

If any provision of these Terms of Service is found to be unenforceable or invalid, that provision will be limited or eliminated to the minimum extent necessary, and the remaining provisions will remain in full force and effect.
